All permanents listed there must be on the battlefield(cleric class must be level 2, so preferably get it out before the rest of the combo), except for Ulamog and Kozilek, who need one of them in the hand or library. Most tutors in the deck tutor to the graveyard for synergy with return to the light realms, which can bring out the whole combo at once. \n\nFading Hope is the Mascot because the chances I ever pull this off are null. We run recursion and tutors to get out the combo under a silence and protect its pieces while assembling it. Everything else is mana gen.\n\nThe Combo:\nAny creature entering triggers Carnival of Souls, causing you to lose 1 life and make B. We will not use the B, so it makes infinite B.\nThe loss of life triggers Oath of Lim-Dul, prompting you to sacrifice something or discard a card. Discard a card. If you are ever low on cards, discard Kozilek or Ulamog.\nDiscarding a card triggers Glint Horn Buccaneer, who pings our opponents for 1 each. Given this is a combo, any player will eventually die to this.\nThe opponents losing life triggers Exquisite Blood, which gains you 1 life 3 times.(Cleric class boosts this to 2 life 3 times). \n\tSub-loop, the lifegain causes cleric class to put a +1/+1 counter on Fathom Mage, who draws you a card, replenishing the one discarded earlier.\nThe lifegain causes our Famished Paladin(enchanted with Samite Blessing) to untap. Before the trigger resolves, we tap Famished Paladin to use the Samite Blessing to target itself(or any other target) and Segmented Wurm. The untap resolves after Segmented Wurm's ability and does nothing, but we mainly needed to target Segmented Wurm.\nSegemented Wurm triggers on being targeted to put a -1/-1 counter on itself.\nFain the Broker(enchanted with Freed from the Real), taps to make a treasure token by removing the -1/-1 counter from Segmented Wurm.\nActivate the treasure token to pay for Freed from the Real to untap Fain.\nCrime Novelist and Krenko both trigger off the artifact sac, order so that Crime Novelist comes first to generate the R then pay the Krenko cost, creating a 1/1 red goblin that triggers Carnival of Souls.\n"}]}
